How to make Performance Point KPI should be a column in Score Card

Hi,

I have 4 error category each has 1 KPI. So totally i have 4 KPI's and one dimension also which is name.

Name should be left side and 4 KPI and corresponding "Actual and Target" should be as a column in Score Card.

How should i make it. Please see the below sample format.
